Program Helps SD Manage Chronic Disease
BETTER CHOICES, BETTER HEALTH® SOUTH DAKOTA
Living with a chronic disease – like diabetes, high blood pressure, or arthritis – can be lonely, confusing, frustrating, and, if not managed correctly, debilitating and affect how long we live. Don’t let your health condition get the best of you. Take control by signing up for a Better Choices, Better Health® (BCBH) workshop. BCBH is designed to help you get back in charge of your life.
BCBH, available to adults in South Dakota, is a community-led evidence-based chronic disease self-management program developed by Stanford University, and is supported by the South Dakota Department of Health, South Dakota Department of Human Services, and SDSU Extension Services.
BCBH workshops bring adults with a variety of chronic conditions together once a week for six weeks to learn skills and tools that will help you gain confidence to better manage your chronic conditions. By participating you feel empowered and responsible for improving your quality of life, and find a supportive environment and a sense of community to help you better meet the challenges and barriers of participating in daily activities while dealing with the emotions that come from living with a chronic condition.
